2010-2015 Service Delivery Plan

Florida Statute 383.216 legislates that Healthy Start Coalitions in Florida, in cooperation with the Florida Department of Health, implement the provisions of Florida ’s Healthy Start legislation within Sarasota County . In carrying out that intent, our Coalition is responsible for developing a maternal and child health care service delivery plan in accordance with sections 10D-113.001 through 10D-113.007 of the Florida Administrative Code.  At a minimum, the Healthy Start Coalition is to:

 

  1. Perform a community assessment of the prenatal and child health care needs of Sarasota County .

  2. Design a prenatal and child health care service delivery plan to enhance

    1. access to prenatal care and delivery services; and

    2. case finding and outreach for pregnant women who are at risk for poor birth outcomes or those and children 0-3 whose proper health and development is at risk. 

  3. Provide high risk services, to include:

    1. coordination of care utilizing a family support plan;

    2. home visiting to provide education and support;

    3. special wraparound services, such as education and counseling for nutrition, smoking, childbirth, breastfeeding, parenting, and interconception health.

  4. Solicit providers for needed services.

  5. Determine allocation of resources.

  6. Build broad-based community support.

The Healthy Start Coalition is currently working on the next Service Delivery Plan for 2010-2015.  Maternal-child health professionals, organizations and agencies, former Healthy Start clients, and the public are invited to participate in the process.  If you are interested, call 941-373-7070 , Ext 301.
